Wild Pokémon never hold an item. Catching one, or using Thief or Covet on one, never gives you an item.
In Emerald, a wild Pokémon of a species that has held-item data carries its common item half the time and its rare item 5% of the time (Compound Eyes in the lead raises both). pkmn does not give wild Pokémon items yet. When it does, item pages will list which Pokémon hold them.
Trainers' Pokémon do hold their items, as in the original games, and the Pickup ability finds items after battles.
