Each chapter covers the stretch between two story milestones: where to go, what to do and the choices on the way, then the wild Pokémon, trainers, items and shops of the places it reaches for the first time. The game data on each page is generated from the release, so it stays in step with what is actually in the game.
The Kanto and Hoenn stories are the two campaigns of one character. The level cap they mention only applies when a run turns on the level cap modifier.
